This study will be conducted in a cohort of consecutively enrolled patients with type 2 DM. The investigator will screen the subjects from outpatient clinics and hospitals. Through review of medical history and lab measurement, the following data will be collected including body weight, waist circumference, blood pressure, HbA1c, fasting blood glucose, fasting blood lipid, urine microalbumin to creatinine ratio in all the subjects who meet the criteria. The data on concurrent and historical medications and treatment patterns will also be collected.

Approximately 25,000 patients will be recruited. The patients will be selected from departments of cardiology, endocrinology, and nephrology in at least 100 hospitals, including tier 3 (primary or teaching hospitals), tier 2 (secondary or city level hospitals), and tier 1 (community hospitals/health centers), in 6 regions including North, Southwest, South, East, West, and Northeast.
